EU AI Act technical documentation generator (Annex IV) — free template for high-risk AI systems
Providers of high-risk AI systems must draw up technical documentation before the system is placed on the market or put into service, keep it up to date, and make it available to national competent authorities and notified bodies (Art. 11(1)). Annex IV of Regulation (EU) 2024/1689 lists what it must contain, in nine sections. Fill them below and download a draft. Nothing you type leaves your browser; the form is autosaved in this browser only.
- Who: providers of high-risk AI systems (Art. 6). Deployers do not write Annex IV documentation; they use the provider's instructions for use (Art. 13) and may need a FRIA (Art. 27).
- SMEs and start-ups may provide the Annex IV elements in a simplified manner; the Commission is to establish a simplified technical documentation form for this purpose, which notified bodies accept for conformity assessment (Art. 11(1)).
- Annex I products: where the high-risk AI system relates to a product covered by the Union harmonisation legislation in Annex I, Section A, a single set of technical documentation is drawn up, containing both the Annex IV information and the information required by that legislation (Art. 11(2)).
- When: under Art. 113, the high-risk obligations apply from 2 August 2026 for systems listed in Annex III, and from 2 August 2027 for systems that are safety components of, or are themselves, products covered by Annex I (Art. 6(1)). Check whether later amendments to these dates are in force before relying on them.
- Retention: keep the technical documentation at the disposal of the national competent authorities for 10 years after the system has been placed on the market or put into service (Art. 18(1)).
Section 3 needs working logs, not a description
High-risk systems must technically allow the automatic recording of events over their lifetime (Art. 12), and deployers must keep those logs for at least six months (Art. 26(6)). Check whether your agent logs meet Art. 12 record-keeping, free and in your browser.
